HOW DO I... Obtain A Repeat Prescription?
If you are taking long-term medication it is unnecessary for you to attend
the surgery on each occasion that you require a repeat prescription. We do ask
you to give us at least two working days’ to process your request and that
you collect it after 4.00pm on the due day to allow the duty doctor to check
and sign the prescription. This should take the form of a request in writing.
You can use the perforated, tear-off, repeat request form that was attached to
your last prescription. If you wish to have your prescription returned by post
please include a stamped addressed envelope. Please remember that prescriptions
handed in on Friday will not be ready until after 4.00pm on the following Tuesday.
We do not normally accept requests for prescriptions over the telephone with the exception of those for patients who are housebound. We check all requests for repeat prescriptions. From time to time you may be asked to attend surgery for review or have a blood test to monitor your condition and the effectiveness of your medication. If a consultant arranges for you to have blood tests to monitor the medication you are taking, could you please ask for a copy to be sent to the surgery.
If you wish, certain chemists will collect your prescription from the surgery to enable you to collect your medication directly from them. You do need to give three days’ notice for this service to allow the chemist to prepare the medication. Please contact your local pharmacy to see if this service is available from them.
HOW DO I... Obtain A Sick Note?
A doctor's note is not required for illness involving absence from
work for seven days or less. A self-certificate which is available
from reception or from your employer is all that is required. If your
employer requests a doctor's certificate for absences of less than
seven days then a private certificate will be issued for which a fee
is charged.
